The Department of Medicine is seeking a physiatrist and medical director for the Rehabilitation, Complex Continuing Care and Transitional Care program.
Working with a multidisciplinary team of rehabilitative therapists, nurses, and physicians, the physiatrist will be responsible for providing consultations and assessments to Osler’s rehab client groups in the areas of rehabilitation, complex continuing care and transitional care. The physiatrist will oversee the inpatient rehabilitation units in collaboration with the MRP. As medical director, the physiatrist will work in partnership with the leadership team to support and drive program goals and priorities in alignment with Osler’s strategic plan. The medical director will co-lead program led initiatives and actively participate on committees to drive operational efficiencies, manage quality assurance, support innovation and community collaboration, and to spearhead high quality patient care, safety and excellence. Remuneration for clinical practice is fee-for-service as set by the OHIP Schedule of Benefits and Fees. Administrative services as the Medical Director, Rehabilitation, Complex Continuing Care and Transitional Care will receive a stipend.
Qualifications
The candidate must have a Certificate of Registration to practice from the College of Physicians and Surgeons of Ontario and have fellowship in Physical Medicine and Rehabilitation from the Royal College of Physicians and Surgeons of Canada. The physiatrist must also be eligible for active staff privileges at William Osler Health System. The position requires excellent communication skills in English. Additional languages are considered an asset as we serve a highly diverse population.
About the program
Osler’s Rehabilitation, Complex Continuing Care and Transitional Care offers the breadth of inpatient rehabilitative care guided by evidence-based best practice and clinical expertise. The rehabilitative care focus is on delivering rehabilitation using a coordinated, interprofessional approach to enable individuals with impairments and disabilities reach and maintain their optimal physical, sensory, intellectual, psychological and social functional levels. In alignment with best practices, Osler’s inpatient units offer rehabilitative care, which differ in terms of the functional trajectory, goals of rehabilitation, rehab intensity and resources, and patient characteristics. The definitions for the bedded levels of rehabilitative care reflect the understanding that the focus of rehabilitative care may vary from where it is a primary focus in some levels (e.g. rehabilitation and activation/restoration) to a more secondary focus in others where the medical complexity of the patient is higher than in other levels (e.g. short and long-term complex medical management). Medical model consists of family physician as the most responsible physician complemented by the nurse practitioner in some care settings (Restorative Care Unit). Our team has experience working with patients who have neurological, medical, orthopaedic and other complex medical conditions.
