COVID-19 Vaccine Clinic

Since our first COVID-19 Vaccine Clinic opened in December, Osler continues to work closely with the Ministry of Health, Ontario Health, and public health units to accept and distribute the vaccine in accordance with Government direction.

In alignment with the Province’s three phase COVID-19 Vaccine Distribution Plan, at this time, we are working closely with Public Health to vaccinate high-risk populations including people and staff in long-term care homes, retirement homes and other congregate living settings for seniors, health care and health-affiliated workers, Indigenous adults and adult members of their household, and most recently, people with highest and high-risk health conditions, and people who are 50+ years of age or older or turning 50 in 2021.

50-59 years of age in ‘hot spot’ communities

Some areas in Peel and North Etobicoke have been identified as ‘hot spot’ communities by the Ministry. We are working closely with Public Health to vaccinate people who are 50-59 years of age (born in 1971-1962) living in identified ‘hot spot’ communities in Osler’s catchment area. Postal codes of the ‘hot spot’ communities are listed below:

  • Peel: L4T, L4W, L4X, L4Z, L5A, L5B, L5R, L5C, L5K, L5L, L5M, L5N, L5V, L5W, L6P, L6R, L6S, L6T, L6V, L6W, L6X, L6Y, L6Z, L7A, L7C
  • North Etobicoke: M9R, M9V, M9W

Special education teachers and education staff

Osler is not vaccinating this group of professionals. Please visit Ontario’s COVID-19 Vaccine Appointment Booking Portal to book an appointment at a Public Health community vaccine clinic near you.

18+ in select ‘hot spot’ communities

Osler is supporting the Ministry of Health, BAPS and other community partners with the appointments-based pop-up Vaccine Clinic located at the BAPS Swaminarayan Complex in North Etobicoke. The BAPS Clinic will administer the COVID-19 vaccine to residents age 18 and older in ‘hot spot’ communities with the following first three characters of their postal code:

  • M9R, M9V, M9W, L4T or L6S

Appointments can be booked online or by phone at 905-494-6685, from 8:00 a.m. - 6: 00 p.m., seven days a week. Vaccinations are by appointment only. Walk-ins will not be seen.

Please note Osler’s vaccine clinic booking phone line is experiencing a high call volume. We are working hard to respond so please be patient with us.

Osler's vaccine clinic appointment status

Our vaccine clinics are currently fully booked. We are working quickly to expand our clinics and their hours to offer more appointments. Please check back in the next few days for more available appointments.

There are a number of community vaccine clinics available. Visit the Ontario’s COVID-19 Vaccine Appointment Booking Portal to book an appointment at a community vaccine clinic near you.

We know this is a very exciting milestone for our community and everyone is eager to get vaccinated. Please be patient and only visit Osler with a vaccine clinic appointment.

Steps to booking your COVID-19 vaccine appointment online

You will need to register for a MyOsler account before booking an online appointment at Osler’s Vaccine Clinic. Only those who are eligible will be able to register for a vaccine appointment.

Step One:

1. To register for a MyOsler account, you will be asked to provide your full name, date of birth, gender as well as your phone number and email address. You can only use one email address per person. Email registration is the preferred method to manage your account.

OR

2. If you do not have an email address, you will have the option to register using a valid mobile phone number. You may use the same phone number to register up to four (4) family members if necessary. Unfortunately, landlines cannot be used to register.

Step Two:

After providing your information and accepting Osler’s privacy policy, you will receive a verification email or text message depending on how you chose to register.

Email Registration: Click on the link in the email to verify your account.

Phone Registration: Enter the 4-digit code sent to your mobile phone to verify your account.

Step Three:

Upon completion, you can sign in and follow the prompts to select the appointment time that is most convenient for you. The appointment for your second dose will automatically be booked based on the time you select. Please ensure you are available to attend both your first and second dose appointments.

Booking your COVID-19 vaccine appointment by phone

Please call Osler’s vaccine clinic booking centre at 905-494-6685 from 8:00 a.m. - 6:00 p.m., seven days a week. Vaccine Clinics across Ontario are currently experiencing high volume of demand so please be patient. Booking online is the fastest way to book at vaccine appointment but we understand going online may not work for everyone.

Eligibility

Who can get the vaccine?

In alignment with the Province’s three phase COVID-19 Vaccine Distribution Plan, at this time, we are working closely with Public Health to vaccinate:

High risk populations currently eligible to receive their COVID-19 vaccination include people who are:

  • 50-59 years of age (born in 1971-1962) living in identified ‘hot spot communities’ in Osler’s Peel or North Etobicoke catchment area
  • 60 years of age and older (born in 1961 or earlier)
  • Indigenous adults and their household members who are 18 years of age or older, regardless of status or Indigenous identity
  • Adult recipients of home care

All health care workers (including custodial, security, reception staff etc.) working (in-person and remotely) who are: 

  • Acute care and other hospital setting (e.g. any actively working Osler or Headwaters Health Care Hospital staff, physicians or volunteers)
  • Community physician/physician specialist/nurse practitioner and office/support
  • Long Term Care homes (including essential caregivers identified by the home)
  • Retirement Homes (including essential caregivers identified by the home)
  • Team supporting outbreak response
  • COVID-19 assessment centres, vaccine clinics, mobile immunization & testing teams, isolation centres and labs
  • Indigenous health services
  • Needle exchange services and supervised consumption sites

High-risk community-based providers serving specialized patient populations (contacted by Peel Public Health to indicate they are on the eligibility list):

  • Community Health Centres, Adult Day Programs, Home and Community Care staff
  • Correctional facilities
  • Senior congregate settings
  • Residential facilities
  • Hospices and palliative care staff
  • Shelters
  • Supportive Housing
  • Dentists, dental hygienists, denturists and their office staff
  • Community-based patient-facing workers in pharmacies (including pharmacists, pharmacy technicians, pharmacy assistants and cashiers working in the pharmacy dispensary)
  • Patient Transfer Services
  • Midwives
  • Laboratory Services
  • Campus health
  • Community diagnostic imaging
  • Registered dieticians
  • Independent health facilities (opticians/optometry, podiatry, audiology, speech-language pathology, medical and surgical specialties)
  • Naturopathy and holistic care
  • Social work
  • Sexual health clinics
  • Non-acute rehabilitation and therapy
  • Chiropractic
  • Chronic pain clinics
  • Kinesiology
  • Occupational therapy
  • Physiotherapy
  • Psychiatry
  • Psychology
  • Psychotherapy
  • Registered massage therapy, acupuncture, other therapy
  • Developmental and disability services
  • Mental health and addictions
  • Canadian Blood Services)
  • Public Health
  • Faith leaders who as part of their regular role are at an increased risk of exposure to COVID-19 through close contact with persons and families at end of life, care of the deceased, funerals, bathing or other ceremony, home visits to unwell persons, pastoral care in hospitals, long-term care or retirement homes or other vulnerable settings

Health-affiliated workers (these groups have been contacted directly with instructions on how to get vaccinated):

  • Firefighters providing medical response
  • Police providing medical first response
  • Child care and school-based health service providers (e.g. psychologists, occupational therapists, social workers, speech-language pathologists_
  • Quarantine officers at Toronto Pearson Airport
  • Metrolinx Special Constables
  • TransHelp

Those with the following highest and high-risk health conditions (and up to one caregiver):

  • Organ transplant recipients

  • Hematopoietic stem cell transplant recipients

  • Neurological diseases in which respiratory function may be compromised (e.g. motor neuron disease, myasthenia gravis, multiple sclerosis)

  • Haematological malignancy diagnosed less than one year

  • Kidney disease eGFR less than 30

  • Obesity (BMI greater of 40)

  • Other treatments causing immunosuppression(e.g., chemotherapy, immunity weakening medications)

  • Intellectual or developmental disabilities (e.g., Down Syndrome)

Eligible individuals with those highest and high-risk health conditions will be identified and contacted directly by their specialists (e.g. oncologist, nephrologist, hematologist etc.) or an Osler clinic with instructions on how to book a COVID-19 vaccine appointment.

Ontario's vaccine supply will not arrive all at once, so distribution will happen in stages and information will change as a result.

Booking a vaccine appointment

Do I need to book an appointment to get my vaccine or can I walk-in?

Vaccinations are by appointment only. Walk-ins will not be seen. We know everyone is eager to get vaccinated and we ask for your patience as we work to ensure timely, safe and efficient access to the vaccine.

I’ve booked my appointment. Can my caregiver, spouse or another member of my household get vaccinated at the same time?

Eligible caregivers can receive the vaccine and are required to book their own separate appointment either online or by phone. This will allow us to plan and distribute the vaccine to everyone in a safe and timely manner.

Osler can only administer the COVID-19 vaccine to eligible priority groups, as directed by the Ontario Government and Public Health.

Osler’s vaccine booking centre screens for eligibility prior to booking an appointment and then patient information is validated in-person at the Vaccine Clinic by asking for proof of age, etc.

I’m having trouble booking my appointment online?

If you receive an error message while trying to book online, please try again using either Chrome, Safari or Edge as your browser or another device.

If you don’t receive a verification email within a few minutes, please check your junk/spam folder.

If you are having trouble booking an appointment, please feel free to call Osler’s vaccine clinic booking centre at 905-494-6685, from 8:00 a.m. – 6:00 p.m., seven days a week or email covax.booking@williamoslerhs.ca for assistance. The clinic booking centre will respond within 24-48 hours of your email request. To avoid duplicate requests, please only send one email.

Can I cancel or change my appointment?

Yes. You can cancel or change your appointment using the same method you used to book your appointment. If you booked online, please cancel online. If you booked over the phone, please call the booking call centre.

If you booked online, to cancel or reschedule your appointment:

  • Select the trash can icon and follow the prompt.

I’m scheduled to receive the vaccine but I now have symptoms of COVID-19. Can I still come to the Vaccine Clinic to receive the vaccine?

No. You can not receive the COVID-19 vaccine if you have any symptoms.

What is MyOsler and why do I need to register for account?

Creating a MyOsler account allows you to manage appointments, cancellations and changes to Osler bookings online.

Why do I have to wait 112 days for my second dose?

On March 9, Osler received direction from Ontario’s Chief Medical Officer of Health to extend the second dose interval for all Health Canada approved COVID-19 vaccines to 16 weeks (112 days).

Increasing the time between first and second doses allows more people to get the benefit of immunity from a first dose while the vaccine supply continues to be limited. We want to assure you that the COVID-19 vaccine continues to be equally effective even with the extension of time between doses.

Where are Osler's Vaccine Clinics?

Your booking confirmation will provide you with further details and instructions like location, parking etc.

  • Chinguacousy Wellness Centre: Eligible Peel residents 50+; Indigenous and their household members who are 18 years of age and older
  • Brampton Civic Hospital: Eligible Peel residents 50+; Eligible individuals with highest and high risk health conditions; Eligible Peel health care workers and health-affiliated workers; Indigenous and their household members who are 18 years of age and older
  • Osler-Humber College COVID-19 Vaccine Clinic: Eligible residents 50+; Eligible individuals with highest risk health conditions; Eligible health care workers and health-affiliated workers; Indigenous and their household members who are 18 years of age and older
  • BAPS Shri Swaminarayan Mandir Vaccine Clinic: Eligible residents 18+ from identified postal codes

Is there parking?

Yes, free parking is available at Osler’s Vaccine Clinic. Parking instructions will be provided in your appointment booking confirmation.

Who administers the vaccine at Osler’s Vaccine Clinic?

Vaccinations are provided by trained clinical staff within a safe and secure environment.

What measures have been put in place to safely provide vaccines during COVID-19?

At Osler, the health and safety of everyone is our number one priority.

Our Vaccine Clinic staff are very careful to prevent the spread of COVID-19 when offering immunization. Examples of safety measures include:

  • All individuals attending or working in the Vaccine Clinic will be asked to wear a mask, clean their hands, and to stay at least two metres (six feet) from others.
  • Individuals with symptoms of COVID-19 should not attend the clinic or receive the vaccine.

Osler Vaccine Clinic staff are prepared to manage an allergic reaction should it occur. If you are concerned about any reactions you may experience after receiving the vaccine, contact your health care provider before booking an appointment.

Preparing for your vaccine appointment

What do I need to bring to the vaccine clinic?

Please try to arrive five minutes before your appointment and remember bring:

  • Proof of age (e.g. Ontario Health Card or another form of government-issued identification driver’s license, passport, Status Card or valid health card from another province).
  • Eligible individuals with highest and high-risk health conditions will be asked to provide a verbal attestation at this time.
  • Eligible health care and health-affiliated workers must bring the following documents to their vaccination clinic appointment: 
    • Government issued photo identification (e.g., driver’s license, health card)
    • One supporting document that shows they are an active health care worker (e.g., ID badge, business card, letter from supervisor on letterhead, pay slip, proof of active membership in a relevant professional college/licensing association like CPSO, CDHO)

Remember to also:

  • Wear a short-sleeve shirt or shirt with sleeves that are easy to roll up.
  • Wear your mask that covers your nose and mouth.

What if I don’t have an Ontario Health Card?

If you do not have an Ontario Health Card, bring another form of government-issued photo ID, e.g. a driver’s license, passport, Status Card or valid health card from another province.

Can I bring someone with me to the Vaccine Clinic?

If you are able to come alone, we ask that you do so. This will allow for physical distancing and support the health and safety of everyone. If you need a caregiver for support, they may accompany you to your appointment. They will be screened and required to wear a mask in the clinic at all times.

Do I need to continue to follow public health measures now that I have received the vaccine?

Yes. Continue to follow the advice of public health officials to prevent COVID-19, such as wearing a mask, practicing hand hygiene, and maintaining a physical distance of two metres from people outside of your household.
