Diabetes Care

Osler’s Diabetes Education Centre offers counselling, interactive classes and programs plus outreach services to bring education opportunities to convenient community locations. Our Centre for Complex Diabetes Care (CCDC) – one of only three in Ontario – provides one-stop access to care for people with complex or advanced forms of diabetes. 

Osler also partners with the Central West Self-Management Program – a tremendous resource for people living with chronic conditions such as diabetes who are motivated to manage their diabetes and improve their health.

Can be made physicians in the communities served by Osler. Please call the CCDC Nurse Practitioner, at 905-494-2260, to confirm that your patient meets the program criteria.

In general, patients referred to the CCDC must:
  • Be 18 years of age or older
  • Have comorbid conditions associated with their diabetes, such as vascular disease, renal failure, neuropathy, impaired vision, mental illness, financial difficulty accessing care
  • Have inadequate glycemic control and/or significant co-morbidities which impact on Glycemic Control; and/or
  • Have had recurrent diabetes emergencies or have been hospitalized with poorly controlled diabetes and higher readmission rate

OTN Retinal Screening Program

The OTN (Ontario Telemedicine Network) delivers health care services and education via two-way videoconferencing systems.

The Retinal Eye Screening program gives medical professionals easy access for their patients with diabetes. Referring physicians send patients to a local OTN site where digital images of the eye are taken and sent to an ophthalmologist for assessment. A report with diagnosis and treatment recommendations is then sent back to the physician. Locations:

Central West Self-Management Program (CWSMP)

The CWSMP is hosted by Osler and funded by the MOHLTC. Six-week Self-Management Programs support people living with many chronic health conditions, including Diabetes. Programs are free to participants and some are offered in multiple languages.
