We collect and store the personal health information of registered Osler patients. This information is used by health care professionals to help provide the best possible care. We are committed to confidentiality and only authorized people can access personal health information.
How do I get a copy of my Osler health record?
You must complete a consent to disclose, transmit, access or examine personal health information form.
This consent must:
- be directed to William Osler Health System
- be signed, witnessed and dated
- specify which visit and clinical information is being requested
- specify to whom the information is being released
- be accompanied by government-issued photo identification and the prepayment fee, if required
Is there a fee to get a copy of my Osler health record?
There is no fee to give your record to physicians or other health care facilities for the purpose of your care. For other purposes, fees may apply:

When will my request be completed?
We will process your request as soon as possible, within 30 days of receiving your completed information.
How do I request a correction to my Osler health record?
If you think your record of personal health information is inaccurate or incomplete, please fill out a request to correct personal health information.
How do I request a copy of a deceased patient's record?
The executor of the estate must complete a consent to disclose, transmit, access or examine personal health information form and produce a copy of the will. If no will exists, a Certificate of Appointment of Estate Trustee is required.
How do I get a Birth Certificate?
Hospitals do not issue Birth Certificates. However, we will provide a proof of birth letter on request. To request a proof of birth, complete a consent to disclose, transmit, access or examine personal health information form. A fee, as shown in the hospital's fee schedule, applies.

How do I get a Death Certificate?
Hospitals do not issue Death Certificates. Please contact the Office of the Registrar General at 1-800-461-2156.
