Complex Continuing Care (Brampton Civic Hospital)
Complex Continuing Care (CCC) is a non-acute part of the health services continuum designed to provide medical management, skilled nursing and a range of interdisciplinary, and therapeutic services to achieve patient identified goals and optimize the quality of life of individuals who have chronic or complex conditions.
Our Complex Continuing Care team is inspired by the very individual needs of their patients and deeply committed to giving them the best possible care.
At Osler, the Complex Continuing Care (CCC) Program is a 25-bed unit that incorporates two distinct services:
Long-Term Complex Medical Management
Often described as “chronic care”, this program provides specialized or technology-based care to patients whose needs, at the time of admission, cannot be met in the community or in a long-term care facility. The goal of this service is maintenance and to slow the further loss of function.
Functional Enhancement Service (FES)
Time-limited, low-intensity rehabilitation, this program is designed for people who have more complex illnesses or disabilities, limited activity tolerance, and may need more medical or nursing care than a higher intensity rehab program can provide. The goal of this rehabilitation program is progression to support the patient to return to living in the community.
Complex Continuing Care is not a permanent stay or long-term care program. Our inter-professional team continually assesses patients to ensure that the person is receiving the appropriate level of care in the right location. When a patient no longer requires the intensity of resources or services provided in CCC, the Discharge Coordinator and team will assist the patient and/or their caregivers to prepare for a transition or discharge to a more appropriate care setting.

At risk for readmission to hospital
