We want to make it easier for seniors to get the health services they need – both at the hospital and in the community. That’s why Osler partners with the Central West LHIN, the Regional Geriatric Program of Toronto, local nursing homes, community agencies and others to provide care and support services for seniors.  Together we can do so much more. And, as part of our commitment to being leaders in senior’s care, we’re planning even more for the future!

We are proud to carry the NICHE designation as a sign of our resolve to provide care targeted at older adults. NICHE (Nurses Improving Care for Healthsystem Elders) is the only national designation indicating a hospital's commitment to elder care excellence.

Through our participation in the NICHE program - the largest geriatric nursing program  in North America - we are able to offer care that is informed and tested by a large network of hospitals. You, in turn, receive care that is adapted specifically for your needs.

Acute Care of Elders (ACE) unit

The nursing staff on this unit have special training in caring for elderly patients.

Emergency Departments

Each of the Emergency Department teams includes Geriatric Emergency Management (GEM) Nurses. Their job is to help assess seniors who may need extra care and support when they go home so that they can avoid repeat visits to the Emergency Department.

Locations:  Brampton Civic Hospital and Etobicoke General Hospital 

Geriatric Mental Health Unit

Care is designed to meet the needs of patients who may be frail, have multiple medical conditions, and different behavioural support needs.

Reactivation Care Centre

The Reactivation Care Centre – Church Site houses a 30-bed Osler unit for patients who no longer need specialized acute hospital services, but would benefit from restorative care in a quiet setting while waiting to transition home or to another care facility in the community. Learn more

Short-Term Assess and Restore (STAR) unit

This inpatient unit i designed for patients over 60 years of age who have had an acute hospital stay and need a short but intensive therapy program to help increase their strength and mobility before they are discharged.  The care team includes experts in geriatric care.

Nurse-Led Outreach Team

Nurse Practitioners (NPs) at Osler partner with long-term care homes (LTCH) in the community to provide timely care to seniors and avoid unnecessary transfers to hospital. Through telephone consultation, visits and assessments, the NPs work with the LTCH team, the residents and families to manage the seniors’ health needs.

Geriatric Outreach Team

This hospital team includes a physiotherapist, occupational therapist, social worker and registered nurses who specialize in caring for seniors living in the community. The team provides in-home non-urgent geriatric assessment to frail seniors who are unable to attend clinic visits. The assessment focuses on the senior’s functional, psychosocial, cognitive and physical health status.

The team serves patients living in the Central West LHIN.

Senior Outpatient Clinic

Osler Geriatricians provide comprehensive geriatric assessments and follow-up for patients. Appointments are by physician referral only.
