Medication Lists

It’s important to keep your personal medication list with you during any of your health care visits. This list will help you to keep track of  prescriptions, vitamins, over‐the‐counter medications, herbs and supplements.

Care that's inspired by you!

Whether you are here for a short visit or a longer hospital stay, we want your experience to be everything you had hoped for – and more. We promise care that is consistently excellent, frequently innovative, and always compassionate.

Osler’s bold new partnerships with other health care providers and community-based services are allowing us to extend care beyond our hospital walls, into the neighbourhoods and residences where you live. Many new, innovative options are already in place and others are soon to come. Our goal is to provide you with exceptional care and supports that respect your language and cultural preferences and, whenever possible, to provide that care at a place and time that works best for you.

The long-term health of our communities and the success of our health care system demands that we think differently about how we plan and deliver health care, health promotion, and disease prevention services. Thank you for inspiring us to challenge conventional thinking, grasp new opportunities, and help shape the future of health care for now and for future generations.

Osler’s top priority remains the health and safety of our patients, visitors, staff, physicians and volunteers. Our hospitals are safe places to come for care, and we are taking every precaution to ensure the health and safety of everyone who come through our doors.

As part of our ongoing efforts to reduce the potential transmission of COVID-19, all patients and visitors are required to wear hospital-issued face masks while in our hospitals. As an inpatient, you will need to wear a mask when:

  • Anyone enters your room
  • You are not able to be 6 feet apart
  • You leave your room (for a walk, an appointment/procedure, etc.)

Important message about patient admissions

As the number of patients we serve increases, we want to ensure all patients receive care safely and quickly. This may mean your care could be transferred to another hospital. Delivering safe, high quality care is our top priority and a transfer would occur only when we are confident care needs could be met by another hospital. Thank you for your cooperation and understanding during these challenging times.
