Together with our health care partners, we strive to provide seamless, effective care that best meets the needs of our community's older adults.

A NICHE designated organization

As a sign of our resolve to provide targeted care for older adults, our hospitals are proud members of Nurses Improving Care for Health System Elders (NICHE). This international nursing education and consultation program is a leader in geriatric care in health care organizations.

Seniors' services

Acute care for the elderly (ACE)

Our ACE team helps patients remain independent after a hospital stay. Using a patient and family-centred approach, our seniors' experts create comprehensive geriatric care plans to support each patient's mobility and function while they are in the hospital.

All ACE team members have specialized training in supporting the needs of geriatric patients. The ACE team includes:

  • physicians
  • geriatricians
  • nurse practitioners
  • registered nurses
  • registered practical nurses and health care aides
  • pharmacists
  • registered dietitians
  • speech-language pathologists
  • social workers
  • discharge planners
  • physiotherapists and physiotherapy assistants
  • occupational therapists and occupational therapy assistants

Geriatric emergency management (GEM) nurses

Each Osler Emergency Department includes GEM nurses. These nurses:

  • assess patients to identify their health, functional, emotional cognitive and social needs
  • link seniors to community services
  • provide education to patients, their families and Emergency Department staff
  • support seamless transitions between the Emergency Department and community health care services

Nurse led outreach team (NLOT)

The NLOT provides care to seniors residing in long-term care homes in the community. They provide evaluations and treatments to prevent unnecessary admissions to hospital, as well as reduce the length of hospital stay.

Geriatric outreach team (GOT)

The GOT specializes in caring for seniors in our community. This team provides in-home assessments to at-risk seniors for whom leaving home is physically, mentally or logistically difficult. Assessments focus on functional, psychological, cognitive and physical health.

The GOT collaborates with geriatricians and the health care team at the Centre for Seniors and Rehabilitation at Peel Memorial to best support seniors.

Centre for Seniors and Rehabilitation

Our Centre for Seniors and Rehabilitation offers a team of health care professionals:

  • geriatricians (physicians specialized in the care of seniors)
  • nurse practitioners
  • registered nurses
  • physiotherapists
  • occupational therapists
  • speech-language pathologists
  • social workers
  • registered dietitians
  • pharmacists
  • team coordinators and clerical staff

This team provides standardized comprehensive geriatric assessments and care plans to support seniors with complex care needs. Depending on your needs, you may benefit from one of our clinics.

Memory clinic

The memory clinic diagnoses and treats seniors with a wide range of cognitive disorders. These include mild cognitive impairment, Alzheimer's disease, Lewy body dementia, frontotemporal dementia, Parkinson-related cognitive dysfunction and stroke-related memory dysfunction.

Behavioural and psychological symptoms of dementia (BPSD)

Our BPSD clinic provides comprehensive geriatric assessments that focus on managing behaviours associated with dementia and/or BPSD.

This clinic treats patients age 65 years or older who show signs of:

  • dementia with symptoms such as wandering, aggression, hallucinations, etc.
  • delirium with behavioural symptoms
  • delirium after a hospital stay (this may require a medication adjustment or review)

This clinic also supports long-term care residents showing dementia-related behaviours.

Falls and frailty clinic

The Falls and Frailty Clinic supports seniors at high risk of injury due to a fall. Assessments include interviews, questionnaires, standardized fall risk and frailty testing, physical examinations and cognitive screening, when appropriate.

This clinic is appropriate for people aged 65 years or older who:

  • have a history of falls;
  • have been at risk of falls in the past year; or
  • show clinical signs of frailty (e.g. slower gait speed, weight loss, fatigue, weakness, sedentary lifestyle with or without cognitive impairment, etc.).

Patients are assessed and receive a coordinated care plan from our team of experts.

Continence clinic

The continence clinic aims to improve seniors' quality of life by:

  • reducing the frequency and severity of urinary and fecal incontinence episodes.
  • preventing complications associated with urinary and fecal incontinence.

This clinic treats conditions including:

  • urinary incontinence
  • fecal incontinence
  • stress, urge, frequency or functional incontinence
  • recurrent urinary tract infection
  • prolapse
  • urogenital atrophy
  • chronic constipation
  • accidental loss of stool

Parkinson's clinic

The Parkinson's clinic provides comprehensive assessments and coordinated care plans focusing on movement disorders.

Conditions treated include:

  • Parkinson's disease
  • Multi-system atrophy
  • Progressive supra-nuclear palsy

Osteoporosis clinic

The osteoporosis clinic aims to manage osteoporosis and nutrition to:

  • decrease the future risk of fractures
  • improve quality of life
  • improve safety for the individual and caregiver

We treat patients with suspected or confirmed low bone-density/osteoporosis, increased fracture risk or a family history of osteoporosis.

Chiropody clinic

The chiropody clinic assesses, treats and prevents foot diseases or disorders of the foot by the registered chiropodist. We treat any foot related conditions at all ages with priority given to seniors and clients with health conditions.

Services offered include:

  • foot assessment
  • patient education
  • disease prevention
  • treatment including routine foot and nail care

Referrals

You do not need a doctor's referral to attend this clinic. Please contact the clinic directly.

Neurological rehabilitation clinic

The neurological rehabilitation clinic treats patients 70 years of age and over with neurological conditions diagnosed within the last year which may include stroke and acquired brain injury.

Services offered include:

  • occupational therapy
  • physiotherapy
  • speech-language pathology
  • registered dietitian
