To make a request under FIPPA, please complete and submit the Freedom of Information request and correction of personal information form.
When you complete this form for general or personal information access requests:
- clearly identify the specific record(s) or information you would like to receive;
- specify dates or time periods, where possible; and
- provide as much detail as possible to help us find responsive records.
When you complete this form for personal information correction requests:
- identify the specific record(s) you would like a correction made to;
- the reasons why the personal information is incomplete or inaccurate; and
- the correction you would like made.
If making a personal information access or correction request for yourself, it must be accompanied by a:
- photo of your valid government-issued photo identification, by itself. (e.g., driver’s license, Ontario photo card, passport)
- photo of you holding your valid government-issued photo identification beside your face to confirm your identity.
Identification provided needs to be issued by a federal, provincial, territorial, or state government authority and valid, that is, not expired.
If requesting access to or correction of personal information on behalf of a patient or visitor, it must be accompanied by a:
- photo of your valid government-issued photo identification, by itself. (e.g., driver’s license, Ontario photo card, passport)
- photo of you holding your valid government-issued photo identification beside your face to confirm your identity.
- consent form, letter or email from the respective person to whom the information pertains
- photo of the person’s government-issued photo identification
For personal information access request for deceased, we require documentation that validates your position of estate trustee or administrator of the estate.
