The process to request information depends on the type of your request:

  • To request a copy of or a correction to a patient health record:
    • Visit health records or call 905-494-2120 ext. 58864
    • You will be asked if you are the patient or if you are requesting a record on behalf of a patient (e.g., as a substitute decision maker, power of attorney, legal counsel, insurance company representative, estate trustee, etc.).
  • To request access to non-health related records or correction to personal information:

Freedom of Information

The Freedom of Information and Protection of Privacy Act (FIPPA) provides a right of access to information held by the hospital, and outlines responsibilities with respect to privacy protection of personal information and right of access to that personal information.

Under FIPPA, we:

  • provide people with access to general records about our functions and operations, such as policies and procedures;
  • provide people with right of access to and correction of their personal information; and
  • protect the privacy of personal information.

In keeping with the principles of FIPPA, we make our operational records publicly available whenever possible. A number of records, such as operational plans, expenses and licenses, are also available under public reporting.

To make a request under FIPPA, please complete and submit the Freedom of Information request and correction of personal information form.

When you complete this form for general or personal information access requests:

  • clearly identify the specific record(s) or information you would like to receive;
  • specify dates or time periods, where possible; and
  • provide as much detail as possible to help us find responsive records.

When you complete this form for personal information correction requests:

  • identify the specific record(s) you would like a correction made to;
  • the reasons why the personal information is incomplete or inaccurate; and
  • the correction you would like made.

If making a personal information access or correction request for yourself, it must be accompanied by a:

  • photo of your valid government-issued photo identification, by itself. (e.g., driver’s license, Ontario photo card, passport)
  • photo of you holding your valid government-issued photo identification beside your face to confirm your identity.

Identification provided needs to be issued by a federal, provincial, territorial, or state government authority and valid, that is, not expired.

If requesting access to or correction of personal information on behalf of a patient or visitor, it must be accompanied by a:

  • photo of your valid government-issued photo identification, by itself. (e.g., driver’s license, Ontario photo card, passport)
  • photo of you holding your valid government-issued photo identification beside your face to confirm your identity.
  • consent form, letter or email from the respective person to whom the information pertains
  • photo of the person’s government-issued photo identification

For personal information access request for deceased, we require documentation that validates your position of estate trustee or administrator of the estate.

Please note a $5.00 application fee is required to begin processing your request.

When your access or correction request has been received, we will contact you to acknowledge your request. A fee estimate may also follow for search and preparation related to access requests. In accordance with FIPPA Regulation 460, fees vary depending on the resources needed to complete your request.

Please note that we are not required to release records until fees have been paid, or the issue of fees has been resolved after an appeal to the Information and Privacy Commissioner of Ontario.

We will process your request as soon as possible, within 30 days of receiving your complete information and issue a decision to you. If we cannot fulfill your request within 30 days we will provide you with additional notice.

  • Freedom of Information requests: Under FIPPA there are limited, specific circumstances in which we cannot provide records, in full or in part. If we are unable to provide records, we will give you the reason(s).
  • Personal information access or correction requests: Under FIPPA, if we are unable to provide the record or are unable to complete the requested correction, we will provide the reason(s). For correction requests you may provide a statement of disagreement which will be applied to the record.

If you wish, you can appeal our decision to the Information and Privacy Commissioner of Ontario within 30 days of our decision on your Freedom of Information access or correction request:
