Diabetes Care

If you live with diabetes, we have resources and support that can help.

Osler's DEC offers counselling and support for people with:

  • type 1 diabetes
  • type 2 diabetes (on insulin or insulin pump)
  • gestational diabetes

We can help you and your family understand diabetes and learn how to self-manage the disease. At the DEC, you can learn how to:

  • test your blood sugar levels
  • take insulin
  • manage medications
  • make healthy food choices
  • lead a more active life

DEC services

A diabetes nurse and dietitian will assess your health and provide education tailored to your needs. Other services include:

  • appointments conducted in-person, virtually or by telephone
  • insulin pump training
  • insulin adjustment classes
  • gestational diabetes classes
  • paediatric diabetes program
  • transitional program for young adults moving from a paediatric program
  • referral to a social worker
  • in-services provided to schools and daycares (in-person or virtually)
  • in-services provided to health care providers (hospital or community)

  • You will need a doctor's referral for DEC services
  • DEC services are available by appointment only
  • Interpretation services are available

Our CCDC provides access to care for adults with complex diabetes. The CCDC:

  • offers access to a team of diabetes experts. This includes an endocrinologist, nurse practitioner, certified diabetes educator, registered dietitian, social worker and pharmacist
  • personalizes care to meet your needs
  • partners with programs and clinics across Osler to ensure you receive coordinated care

Any family doctor or medical specialist in the community we serve can refer you to the CCDC.

This free virtual workshop will show you how to reduce your risk of developing type 2 diabetes. It includes interactive lessons, food demonstrations and physical activities led by a registered dietitian and a registered kinesiologist. Learn more about our cooking demonstrations.

If you have prediabetes or are at high risk for developing type 2 diabetes, this is the program for you! No referral is needed. Sign up for the Sun Life | Osler Healthy Active Living Initiative.
